"Billing Provider Address" Rejection Notices
With the conversion to ANSI v5010, the billing provider address must be a complete street address and can no longer be a P.O. Box or lock box. Complete is defined as providing a physical street number address including the full 9-digit, or ZIP+4, ZIP code—this is the traditional 5-digit code plus the extra four digits for localized mail delivery.
Some letters have been incorrectly sent to providers indicating that the billing address on the claim does not match the address on file; however, the claims were originally submitted correctly. We are working to modify internal processes and documentation and apologize for any inconvenience this may have caused.
Please resubmit any claims that are associated with this letter.
