What are PPO, HMO and EPO Group Insurance Plans?
- PPO, or Preferred Provider Organization, plans usually carry a higher monthly premium while allowing your employees to choose any physician they wish, either inside or outside the BCBSNM network.
- HMO, or Health Maintenance Organization, plans facilitate coverage through a primary care provider (PCP) from your employee’s choice of physicians within our network.
- EPO, or Exclusive Provider Organization, plans allow your employees to seek services from a predetermined list of in-network providers without having to seek a referral from their primary care physician.

HMO BlueSM
Best Fit: 51-150 and 151+ Employees
HMO Blue offers your employees a choice of Primary Care Providers (PCPs) and coverage for preventive health services, access to the most comprehensive provider network, and predictable copays.
Blue HMO NetworkSM

BlueNet “H” EPOSM
Best Fit: 51-150 and 151+ Employees
Your employees can choose their own in-network provider. They’ll also have coverage when traveling, including the unique BlueCard® program. The plan also provides comprehensive coverage with simple cost-sharing features, prescription drug coverage, including retail and mail-order services, and additional features.
Blue Preferred EPOSM
