Disease Management – Are you at risk or do you have a chronic condition such as asthma or diabetes? Our team is ready to help. We will work with you and with your health care providers.
Care Coordination and Complex Care Coordination – If you have health issues, Care Coordination can help you manage them. You can learn about plan benefits available to you. You can also learn about health care services you may need. Care Coordination includes Case Management. If you are experiencing the following, our Complex Care Coordinators may be able to help you:
- Multiple health issues
- A transplant
- A complex illness
- A high number of emergency room visits or hospital stays because your condition is not under control
The goal of Complex Care Coordination is to improve your overall health. A provider’s referral to the program is not needed. You or your caregiver can self-refer. You can end the program at any time.
CareLink New Mexico – CareLink is a program to help you improve your health, prevent illness, and learn how to manage illness. CareLink providers can work with you to make a care plan to help you get the medical care, behavioral health and social services you need. This can help you to feel better and meet your goals.
Best BenefitsSM Program – For those who are disabled, there is government assistance. Learn what you qualify for and how to access the programs. We want you to get the help you need.
24/7 Nurseline – If you have questions, our nurses have answers. This help line is staffed 24 hours a day with registered nurses. You can get answers to your health questions when you need them. If an unexpected medical situation arises, a nurse can help you decide if you should call your doctor, visit the ER or urgent care, or treat the problem yourself.
Digital Mental Health – Do you struggle with mental health concerns? If you deal with anxiety, stress, depression, substance use, panic or sleep problems, there’s an online resource with programs that can help. Learn to Live is confidential, and you can access it from anywhere. Programs are based on therapy techniques that help people feel better. Learn to Live is available at no cost to you and your family (ages 13 and older). To start, register at the Learn to Live website (Access Code: NMMED). To learn more, check the brochure.
Mental Health Hotlines – These hotlines are available 24/7. Calls are confidential and at no cost to you.
- 988 Suicide & Crisis Lifeline – provides support for those who are in distress, as well as prevention and crisis information
- National Maternal Mental Health Hotline – provides support before, during and after pregnancy for those who feel overwhelmed, anxious or depressed
Hepatitis C Awareness – How do you know if you have Hepatitis C? You can have it for months or years and not know it. Testing is the only way to know if you have it. If you have this virus, you can get help. Drugs that treat it are covered for members who meet certain requirements.
Maternity Services – Once you are sure you are pregnant, you may choose either your PCP or another Turquoise Care network provider to provide maternity care. The care of a pregnant mother is important and the mother’s health can affect the health of her newborn. When you call, we will:
- Help you choose a primary OB/GYN provider or certified nurse midwife for your pregnancy
- Help you enroll in a Care Coordination program
- Help you choose a PCP for your baby (if your baby is eligible for Turquoise Care coverage)

Ombudsman Specialist – Are you having a difficult time trying to coordinate your health care? Are you unable to get services that you believe are covered under your plan benefits? The Ombudsman Specialist may be able to help. An Ombudsman Specialist can speak up for your rights and help you receive the benefits of your Turquoise Care health plan. The Ombudsman Specialist will look into and address member issues without taking sides. Following New Mexico's Medicaid guidelines and using BCBSNM resources, the Ombudsman Specialist will work to resolve your issues.
Don't forget to talk with your PCP about how to stay fit and eat right. There are recommended health screenings for adults and kids. Here are guidelines to help you manage your care. Ask your doctor about what screenings you might need.
