Develop a Quote
Work with your BCBSNM account executive or use the "Quote a Group" eSales tool in your BAP account . The quoting tool allows you to create quotes for group plans.
Get New Sales Forms
Go to Form Finder. Click on Mid-Market Group from the left menu. You'll need to complete the following:
Employer Group Information (EGI)
Submit required documentation on group size, MSP and COBRA status.
Producer Transmittal Form
Submit this form when applying for coverage
Submit an Enrollment Request
For member enrollment, use Smart Census OR the Group Enrollment Application/Change Form.
Smart Census Import Tool
Upload member enrollment, submit premiums, respond to requests and more.
